To get started, please download and complete our Request for Support Form.
Once completed, return the form to us by email or post. If applicable, please also include a copy of your letter confirming that your child has been accepted onto the pathway for an Autism Assessment.
Important:
All sections of the form must be completed, including the “Getting to Know Your Child” section, before we can process your registration.

What happens next
After we receive your completed form and documents, we’ll begin registering you with the Hub. Once registered, you’ll have full access to:

Groups and activities (including our Primary Fun Session and DUGOUT Youth Group for secondary-aged young people)
